or by mailing a request to:

The cost for a certified birth certificate is $10.00.
Please make checks payable to: Burlington City
Clerk.
A request by mail will be processed and sent
to you within 2-3 days
from receipt.
Please mail the following:
- Your check or money order made payable to:
Burlington City Clerk
- A self-addressed
stamped envelope
- Include the full birth
name and date of birth on the request
- Include the mother's maiden
name and father's name
Identification is not
necessary, as these are public
records.
