The death certificate will be on file in Burlington, if:
1) the death occurred in Burlington, or
2) the deceased resided in Burlington and the death occurred within the state of Vermont.
Otherwise, you may obtain a death certificate from the State Public Records Division:

The cost for a certified death certificate is $10.00
A request by mail will be processed and sent out the day it is received at the Clerk Treasurer's office. You will need the following:
Your check or money order should be made payable to: City of Burlington.
Enclose the payment along with your request.
Self-addressed stamped envelope.
State the full name and date of death on the request.
ID is not necessary.
